I have a Kerbal stranded in a lander orbiting the moon. I had sent a ship into orbit, and I docked the lander to it. And sent the ship to the moon. But I miscalculated the fuel in the lander. When I was taking off from the moon I had realized I only had enough to make a orbit around the moon and not a rendezvous with the main ship.

And so I sent the ship back to Kerbin, leaving the poor Kerbal stranded around the moon until I send something to rendezvous and save him.
