The Tessera Sanitaria

The Ministry of Public Health (Italian: Ministero della Salute) is the ministry of the Government of Montevento responsible for establishing public health policies and strategies, in order to contribute to the improvement of the health of the inhabitants of the Nation and to assure the citizens' right to health protection. The Ministry is headquartered in the Palazzo Maggiolo in San Giovanni, named after one of Montevento's most important and recognized doctors.

Healthcare is provided to all citizens and residents by a mixed public-private system. The public part is provided by the Istituto Nazionale di Sanitą Pubblica (INSP - National Institute of Public Health) which is organized under the Ministry of Public Health and financed through tax funding. The INSP is essentially free except for small, often symbolic co-payments in some products and services. Meanwhile the private system is composed of numerous private health providers, many of which specialise on certain types of treatments not offered, or only partially offered by the INSP.

The National Health Insurance (ASN - Assicurazione Sanitaria Nazionale) seeks to unify the services of both the public and private systems in order to ensure universal access to all quality treatments. Through the ASN, a citizen can have access, in addition to the INSP, to any of the private health providers, allowing them to switch between the different providers according to the treatment they need or to their personal preferences. The following citizens are elegible to access to the ASN:

1) All legally registered employees who work at least 13 days a month or more than 104 hours per month.

2) All employees of the State regardless of how many days or hours they work per month, including members of the Armed Forces and the Government.

3) All registered students who attend classes regularly and show progress in their educative careers.

4) All those receiving a state pension in Montevento.

5) Family members of anyone covered by the ASN who are under the age of 18 and over 18 if they are registered as disabled, as well as spouses/concubines of anyone covered by the ASN.

The ASN is entirely funded by tax funds and through the revenue obtained by services offered by the INSP to private institutions. Each private health provider who is part of the ASN receives a cut of the total tax funds proportional to the amount of services provided yearly and the base cost of them, in order to promote competition to offer the best and most efficient service.

⠀⠀◆⠀ Tessera Sanitaria

Every citizen who has access to the ASN is given a Tessera Sanitaria, which is a card that contains biographical data and welfare information, granting the holder the right to obtain any health service throughout Montevento. Holders of a Tessera sanitaria also can get access to their medical history and other files using the code (codice) printed in it, and use it to obtain discounts of up to 25% on medication and pharmaceutical products at registered pharmacies.

On some occasions, a Tessera Sanitaria can also be given to people of a different nationality than Monteventani, in accordance with current international treaties or collaboration treaties with international organizations that seek to protect their employees during their stay in the Most Serene Republic. It is important to note that, by Law N°11.833, someone without a Tessera Sanitaria will not be denied emergency services at any hospital (public or private), although they may be charged for it later.

The application to request a Tessera Sanitaria can be carried out at any of the offices of the Ministry of Public Health, at any INSP office or at any hospital in the country, be it public or private. It can also be requested online on the website of any of the institutions mentioned above. The procedure has no cost, and the Tessera Sanitaria is delivered in a period of 10 to 20 days at the applicant's place of residence. It does not need any activation, and can be used immediately upon receipt.

The card is valid for a maximum of six years. However, the Ministry of Public Health will automatically send a new Tessera Sanitaria to all those who are still eligible to obtain it if they already had one, unless otherwise stated. Starting in 2021, all data from the Tessera Sanitaria began to be incorporated into the Monteventani Smart Identity Card in order to unify citizens' documents into a single document of universal use.

The National Institute of Public Health (INSP) (Italian: Istituto Nazionale di Sanitą Pubblica) is the state provider of health care in Montevento. It has an extensive network of services throughout the country, being the only health care provider to be present in all of the five provinces of Montevento, as well as in all of the major cities of the country.

It was created in 1927 by orders of the Doge Mauro Spalla and later reformed completely in 1982. The administration of the INSP is presided by a five-member board of directors, who are appointed by the Doge of the Republic in agreement with the Council of Ministers, with the prior consent of the National Council. Of the five members, one represents the workers of the organization and other represents its users. Currently, its General Director is Fernando Senatore, a famous and prestigious oncologist from the Province of Costa Grande.

All the services provided by the INSP are completely free of any cost, except for rare procedures of high complexity and for payments of small amounts of money that are usually made symbolically. All citizens and residents of Montevento have the right to access to the services of the INSP at any moment. Tourists and foreigners also have the right to be cared for at the INSP, although such people must pay for the services they require unless their country of origin has an agreement with Montevento on health. As of 2012, all foreign diplomats in Montevento also have total access to the INSP.

According to data from 2017, the INSP is the health service provider of first choice for 43% of the country's citizens, although it routinely provides its services to more than 70% of the population, most of whom access through the ASN, especially for the treatment of eye conditions and of various types of cancer, as it is the leading institution in those areas in Montevento. Since 2001, it also has a wide service of medical consultations by telephone and sending doctors to homes for emergency situations or routine consultation.

⠀⠀◆⠀ Main Hospitals of the INSP

San Giovanni Central Hospital
Province of Costa Grande - San Giovanni

The San Giovanni Central Hospital (OCSG) is one of the biggest hospitals in Montevento. It was built in 1966, although it was expanded in 2001 to more than double its capacity. It is one of the hospitals with the most modern equipment in the Republic, and it is usually taken as a quality and efficiency reference for the rest of the country's hospitals.

It is a multi-use Hospital conditioned for the treatment of multiple conditions, with a total capacity of 1100 beds, of which 200 are equipped for patients requiring intensive care. It has sectors of dental treatment, cardiology, ophthalmology, hemotherapy, oncology, otorhinolaryngology, imageology, among others. It has its own central laboratory for clinical analysis and also an infectious disease control center.

The OCSG is also used as an University Hospital, where various students from the Faculty of Medicine of the Universitą Pubblica di Montevento are trained and many medical investigations are carried out.

Vaccarelli Psychiatric Hospital
Province of Costa Grande - San Giovanni

The Vaccarelli Psychiatric Hospital is the only psychiatric hospital in Montevento that deals with patients of moderate to high severity. Since its inception it has been dedicated to the diagnosis and treatment of psychiatric illnesses, having its origins in 1870 when it was called as the "National Asylum". During the 1890s, the hospital was known to be one of the best hospitals in South America in the context of mental health, reaching more than 1500 patients at its peak. In 1931 it was acquired by the INSP and renamed to its current name in honor of Aurelio Vaccarelli, a Monteventani physician and psychoanalyst, who was a strong defender of the idea that having a "mental illness" does not make the patient "less than a human", revolutionizing the treatments provided in the hospital by making them more humane.

The average age of the patients of the Vaccarelli Hospital is 30 years, more than half of whom arrive by court order. 42% suffer from schizophrenia while 18% suffer from depression and mania; there is also a high percentage of patients addicted to coca paste and cocaine. It employs approximately 610 civil servants, psychologists, psychiatrists, social workers, administrators, and guards, among others.

Delfina Sozio Women's Hospital
Province of Fiume D'Oro - Nuova Firenze

The women's hospital, named after the first woman to receive a medical degree in Montevento, was built in 2011 and consists of the newest hospital belonging to the INSP. It is in charge of providing care in the specialties of gynecology-obstetrics and newborn care, it has a Human Milk Bank and a Home for Mothers. It is a reference in the treatment of various diseases mainly related to women, such as breast cancer, postmenopausal osteoporosis and a wide variety of urinary infections. Although its name may indicate otherwise, it also offers various services to men, especially to single fathers, who are offered support regarding the health care for their children.

The Women's Hospital is also in charge of promoting and teaching contraceptive methods for both women and men and is one of the few institutions in Montevento capable of carrying out abortions (in accordance with the provisions of the Law, which stipulates that abortions should only be carried out in case the life of the fetus or the mother is endangered, or when other well-founded causes occur, such as rape, emotional instability of the mother or lack of financial resources).

>> OTHER DEPENDENCIES OF THE MINISTRY OF PUBLIC HEALTH

The Ministry of Public Health is also in charge of other institutions and organizations linked to the field of health. All these organizations report directly to the Minister of Public Health, who is in charge of their supervision and control at all times, having full power to replace their Directors and officials at any time. The Doge of the Republic also has full authority over these organizations. Among the most prominent organizations dependent on the Ministry it is possible to find the National Administration of Drugs (ANF) and the National Institute for Donation and Transplantation of Cells, Tissues and Organs (INDT).

National Administration of Drugs - Amministrazione Nazionale dei Farmici - ANF

The ANF is an organism dependent on the Ministry of Public Health that was created in 1992 to meet the needs of the growing pharmaceutical industry in Montevento. The organism controls and guarantees that medicines, foods and medical devices have efficacy, safety and quality. For this, it carries out the processes of authorization, registration, regulation, surveillance and inspection of the products of its competence throughout the national territory, ensuring compliance with the legal, scientific, technical and administrative provisions included within the scope of Monteventani regulations.

The products currently regulated by the ANF are: medicines of any type, foods, medical products, diagnostic reagents, cosmetics, dietary supplements, sanitary products for domestic use, oral hygiene products for dental use and biological products. Any national or international company that wishes to commercialize products in any of these categories in the territories of Montevento, must request a permit from the ANF, which will study its safety and quality and grant, if it considers it appropriate, the permission required.

The ANF is also in charge of annually publishing all the reference standards for medicines, food ingredients, dietary supplements and their ingredients in the Monteventani Pharmacopoeia, which also has information regarding how the different quality analysis should be carried out for each product to be considered suitable for the market. All products within the categories in which the ANF is competent must follow the guidelines established in the Monteventani Pharmacopoeia, although the European Pharmacopoeia is also accepted.

The authorization and approval of laboratories is also within the ANF's competencies, in such a way as to ensure that the conditions of hygiene and safety of workers are adequate for the production of different pharmaceutical and cosmetic products. Those products that are manufactured within Montevideo in laboratories without ANF approval are considered of insufficient quality.

National Institute for Donation and Transplantation of Cells, Tissues and Organs - INDT

The Istituto Nazionale per la Donazione e il Trapianto di Cellule, Tessuti e Organi (INDT) was created in 2009 to provide comprehensive care in the field of cell, tissue and organ transplants, including regenerative medicine to the entire Monteventani population, in accordance with legal and ethical principles and current scientific knowledge, strengthening and coordinating the transplant activity in Montevento to improve the health of the population.

Its main function is to act as a bank of tissues, organs and cells, obtained from organ donors. It has the capacity to store and conserve organs, in addition to responding quickly to requests to distribute the different organs to Montevento's hospitals as soon as they are required, without compromising their quality. It is also responsible for the preparation of the waiting lists to access the organs, classifying potential recipients according to the urgency with which they require a donation, always prioritizing those who are at risk of losing their lives.

Since 2009, all citizens of Montevento are considered potential organ donors when they die, and instead must inform the INDT if they wish not to be considered as one, in order to be added to the National List of NOT donors. If a person who is not on that list dies, and meets all the necessary criteria to be an organ donor, the INDT is allowed to extract any organ, tissue or cell from the body to be used for medical transplants or for scientific research, regardless of the opinion of family members or friends of the deceased person.

The INDT also operates the Immunogenetics and Histocompatibility Laboratory of Montevento, which has as a central task to carry out immunogenetic studies of recipients and donors of solid organs and bone marrow. These studies are necessary to evaluate and monitor the immune response that is triggered by an implant or transplant, that could cause the rejection of the new organ if treated incorrectly.
